All healthcare organizations are responsible for providing the best care possible to their patients. While accidents do happen, there is evidence to suggest that many adverse events are preventable. The use of safety practices such as crew resource management, computerized medical order entry, and bar coding are some strategies that could avoid medical and safety errors (Mitchell, 2008). All healthcare managers should take action to avoid common but dangerous patient safety problems such as healthcare-associated infections and hygiene problems. In 2014, deaths from medical errors reached an all-time high of 400,000 people per year and cost the United States nearly $1 trillion (McCann, 2014). Avoiding medical errors is a team effort and is established within a safety-oriented culture within a hospital. Communication between staff and strong leadership can ultimately make these unnecessary events such a thing
