Embedding strong primary care providers who can lead patient care coordination efforts and build on the patient-centered model will be a key focus for nurse leaders. The nurse manager will assist the organization in capitalizing on coordination efforts outside of the primary care practice by obtaining additional health care providers that include specialists in synchrony to focus efforts in providing quality care that delivers favorable outcomes centered on the patient.
